Using Health Insurance: Psychotherapy is usually covered by health insurance, to the extent your policy offers mental health benefits. Please contact your provider before our first session to determine whether or not you will need a treatment plan and/or session authorization. Provide me with a copy of the required treatment plan and the provider's mailing address/fax number. I will complete the treatment plan and submit on your behalf. Using Medicare: I am a Medicare provider. If Medicare is your primary health insurance, I accept the fee which Medicare determines for my service. I submit all bills directly to Medicare. After Medicare pays their portion of the bill, your Medigap/Secondary insurance will pay their portion of the bill. Please contact your Medigap/Secondary insurance to verify that they will cover the non-Medicare reimbursable portion of my services.
